The episode begins with Kaneki being taken to a hospital, where he meets Dr. Akihiro Tsukiyama, a doctor who is revealed to be a ghoul. Tsukiyama is friendly and helpful, but Kaneki is still wary of his intentions.

As Kaneki navigates the complex world of ghouls, he begins to question his own identity and the nature of good and evil. He also starts to develop feelings for Rize, the ghoul who saved his life.

Meanwhile, Nishio, a young ghoul who Kaneki met earlier, is shown to be struggling with his own identity as a half-ghoul. He is mocked and belittled by other ghouls for his half-human appearance.
